Persyaratan Pengajuan Kartu Istri / Suami (KARIS/KARSU) :
1. Formulir Laporan Perkawinan Pertama – download file *)
2. Fotocopy Surat Nikah
3. Fotocopy SK CPNS *)
4. Fotocopy SK PNS 100%  *)
5. Fotocopy SK Terakhir  *)
6. Fotocopy SK Konversi NIP *)
7. Foto Suami / Istri berwarna (2×3 cm) sebanyak 4 lembar
*) jumlah 2 lembar dilegalisir PPK (Pejabat Pembina Kepegawaian)

KARTU PEGAWAI(KARPEG)

Persyaratan Pengajuan Kartu Pegawai (KARPEG) :
1. Fotocopy SK CPNS *)
2. Fotocopy SK PNS 100% *)
3. Fotocopy SK terakhir *)
4. Fotocopy SK STTPL (Surat Tanda Tamat Pendidikan dan Pelatihan) *)
5. Fotocopy SK Konversi NIP *)
6. Foto hitam putih (2×3 cm) sebanyak 4 lembar
*) jumlah 2 lembar dilegalisir  PPK (Pejabat Pembina Kepegawaian)

Askes Asuransi Kesehatan (BPJS)

Persyaratan Pengajuan Asuransi Kesehatan (ASKES) :
1. Blanko Formulir (bisa diambil di Bagian Kepegawaian)
2. Fotocopy SK CPNS *)
4. Fotocopy Surat Nikah
5. Fotocopy Akte Kelahiran Anak
6. Fotocopy daftar gaji
7. Fotocopy SK Konversi NIP *)
8.  Fotocopy KTP
7. Foto berwarna (2×3 cm) sebanyak 2 lembar
*) dilegalisir  PPK (Pejabat Pembina Kepegawaian)
